Common use
Astelin Nasal Spray is primarily indicated for the treatment of symptoms associated with seasonal allergic rhinitis (hay fever) and perennial allergic rhinitis in adults and children 5 years of age and older. These symptoms include rhinorrhea (runny nose), sneezing, nasal pruritus (itching), and nasal congestion. Some healthcare providers may also prescribe Astelin off-label for vasomotor rhinitis and as adjunctive therapy for nasal polyps. The medication works locally in the nasal mucosa to block histamine receptors and inhibit the release of inflammatory mediators from mast cells.
Dosage and direction
For adults and children 12 years and older: 1 or 2 sprays per nostril twice daily. For children 5 to 11 years: 1 spray per nostril twice daily. Prime the pump before first use by releasing 4 sprays until a fine mist appears. Gently shake the bottle before each use. Blow nose gently to clear nostrils before administration. Tilt head slightly forward and insert tip into nostril, pointing toward the outer wall of the nose. Close the opposite nostril with finger and spray while breathing in gently through the nose. Repeat in other nostril. Avoid blowing nose for 15 minutes after administration.
Precautions
Do not use if allergic to azelastine hydrochloride or any inactive ingredients. Use with caution in patients with renal or hepatic impairment. May cause drowsiness in some patients; exercise caution when driving or operating machinery. Avoid alcohol consumption as it may enhance CNS depression effects. Not recommended for children under 5 years of age. Contact healthcare provider if symptoms worsen or do not improve after 3 days of use. Inform your physician if you are pregnant, planning to become pregnant, or breastfeeding.
Contraindications
Hypersensitivity to azelastine hydrochloride or any component of the formulation. History of severe adverse reaction to any antihistamine medication. Patients with narrow-angle glaucoma. Those with urinary retention, especially in men with prostatic hyperplasia. Severe renal impairment (creatinine clearance <30 mL/min). Concurrent use with monoamine oxidase inhibitors (MAOIs).
Possible side effects
Common side effects (≥2%): bitter taste (19.7%), somnolence (11.5%), nasal discomfort (4.1%), headache (3.6%), pharyngitis (2.0%). Less common side effects: epistaxis (nosebleeds), nasal dryness, sneezing paroxysms, dizziness, nausea, weight gain, myalgia. Rare but serious side effects: anaphylaxis, bronchospasm, facial edema, tachycardia, hypertension. Most side effects are mild to moderate and often diminish with continued use.
Drug interaction
May enhance CNS depression when used with alcohol, sedatives, tranquilizers, or other CNS depressants. Potential interaction with MAO inhibitors (contraindicated). Possible increased risk of QT prolongation when used with other QT-prolonging drugs. No significant interactions identified with oral contraceptives, warfarin, or digoxin in clinical studies. Always inform your healthcare provider about all medications, including prescription, over-the-counter, and herbal products.
Missed dose
If a dose is missed, take it as soon as remembered unless it is almost time for the next dose. Do not double the dose to make up for a missed one. Resume regular dosing schedule. If multiple doses are missed, contact your healthcare provider for guidance. Consistent daily use provides optimal symptom control.
Overdose
Symptoms may include drowsiness, dizziness, nausea, vomiting, and headache. In case of suspected overdose, contact poison control center (1-800-222-1222) or emergency room immediately. Treatment is supportive and symptomatic. There is no specific antidote for azelastine overdose. Hemodialysis is not likely to be effective due to high protein binding.
Storage
Store upright at room temperature (15-30°C or 59-86°F). Do not freeze. Keep bottle tightly closed and protected from light. Discard after 200 sprays (as marked on bottle) or 3 months after opening, whichever comes first. Keep out of reach of children and pets. Do not store in bathroom where moisture could affect medication stability.
